How do I book an appointment?

Do you offer evening appointments?

Yes. Each clinician has a different schedule and several clinicians offer evening hours.

Do the practitioners offer consults?

Yes. Our intake coordinators can answer many questions, and if you have additional questions, we can set up a 5-10 minute phone consult with the practitioner of your choice.

What to expect in the initial evaluation? 

The initial appointment is either 45 min or 1.5 hours depending on your preference and clinician schedule availability.  Please ring the bell and have a seat in the waiting area and your clinician will call you in. 

The initial evaluation typically consists of two sessions with adults and two to three sessions with a child.  During the initial appointment you will have the opportunity to describe your health concerns and provide any information that you would like to share.  The clinician will explore with you your current condition, your medical and mental health history, family history, social history and other relevant topics.


You have the right to privacy. All your personal information and records are kept confidential. We are committed to protecting your privacy and will not release your information without prior written consent. The only situation where we may be required to compromise your confidentiality is in the case of emergency, a situation of imminent danger to your life or the life of another person.


In the initial appointment we will talk about your current concerns and key background information. Please bring any prior records you would like to share, a list of prescription and over the counter medications, and the intake forms if you have not completed them online.


In the initial appointment, we will meet with the teen alone for part of the time and with the parents alone for part of the time. Please bring any prior records of any psychological testing or prior evaluations, a list of prescription and over the counter medications, and the intake forms if you filled them out before the appointment, and the names of your pediatrician and any other health care providers.


The initial appointment is with parents only and the second visit is with your child. Please bring any prior records of any psychological testing or prior evaluations, and the intake forms, and a toy for your child. The third appointment is an opportunity for feedback and treatment planning with the parents only.

We welcome your interest. We aim to provide the best care possible to you, and to schedule as soon as we are able. Please be advised that the initial evaluation process takes time, and we may not be able to accommodate urgent appointment requests. We are an out-of-network provider. If you have out-of-network benefits for mental health, we will contact your insurance company regarding your elligiblity for reimbursement.  Average appointment cost is $200.  Appointments with Dr. Lewis are higher cost than with our psychotherapists and psychiatric nurse practitioners.  Sliding scale rates are available on case by case basis.

Can you tell me what my insurance will pay before I book an appointment?

Insurance reimbursement is specific to your individual plan, so we’re unable to give estimates until we verify with your insurance after your first visit with us.  We are available to answer your insurance-related questions to the best of our ability. What’s your cancellation policy?

We have a 48 hour cancellation policy. For Monday appointments we request cancellation by Friday. This enables us to notify patients on the waitlist that appointments have become available and to decrease your wait time for appointments. We do not double book and we run on time, please understand our need for a 48 hour cancellation policy.
