Spravato® (esketamine) nasal spray is FDA-approved for adults with treatment-resistant depression, used on its own or with an oral antidepressant. It is also approved, together with an oral antidepressant, for depressive symptoms in adults with major depressive disorder who have acute suicidal thoughts or behavior. It is one option for people who have not had enough relief from standard antidepressants.
How Spravato® Works
Spravato® is an NMDA receptor antagonist, which sets it apart from traditional antidepressants that primarily target serotonin, dopamine, or norepinephrine pathways.
While esketamine is derived from ketamine, they are not the same. Spravato® is a specific FDA-approved nasal spray product for the depression indications above, and it is only given under professional supervision in a certified healthcare setting.

What to Expect During Spravato® Treatment

Initial Dosing and Observation Protocol
Spravato® is administered as a nasal spray in a certified clinical setting. You are monitored for at least two hours after each dose for sedation, dissociation, breathing and blood pressure. Do not drive or operate machinery until the next day after a restful sleep, so plan a ride home.

Treatment Schedule
The treatment usually begins with twice-weekly sessions that taper down as you progress. This schedule supports gradual improvement while allowing your care team to continuously evaluate and adapt your plan.

Not for Use as an Anesthetic or Pain Reliever
Spravato® is approved only for the depression indications described above. It is not approved as an anesthetic or for pain relief.
Not a Substitute for Crisis or Emergency Care
Spravato® is approved, with an oral antidepressant, for depressive symptoms in adults with major depression who have acute suicidal thoughts or behavior, but it has not been shown to prevent suicide and it does not replace emergency or hospital care. Not Approved for Use in Children
Per FDA guidelines, Spravato® is only approved for adults and should not be used in patients under 18 years old.

Spravato® carries FDA boxed warnings for sedation, dissociation, respiratory depression, abuse and misuse, and suicidal thoughts and behaviors in young people. For that reason it is dispensed and dosed only in a certified medical setting, with at least 2 hours of monitoring after each dose.
