Ever feel like you’re running on empty no matter how much you sleep? Or find yourself forgetting things that should be easy to remember? You might be experiencing the hidden effects of vitamin B12 deficiency—a condition I see in my practice far more often than most people realize.

Hi there! I’m Dr. Bliss Lewis, and today I want to share why I’m so passionate about addressing B12 deficiency differently. While conventional medicine often just hands you a supplement and sends you on your way, I’ve seen firsthand how digging deeper for the real causes can transform lives.

 

Why Your B12 Matters More Than You Think

Vitamin B12 isn’t just another nutrient on a long list of vitamins. It’s absolutely essential for:

  • Creating the energy that powers every cell in your body
  • Protecting your nerve cells (including your brain!)
  • Building healthy red blood cells that carry oxygen
  • Supporting your DNA and genetic expression

When B12 levels drop, your entire body feels the effects—often in ways you wouldn’t expect.

One of my patients, Sarah (name changed for privacy), came to me after seeing three different doctors. She’d been told her fatigue, brain fog, and tingling hands were “just anxiety” or “probably stress.” A simple test revealed severe B12 deficiency, and within weeks of proper treatment, she felt like herself again.

 

Finding Your Root Cause: The Detective Work

What makes functional medicine different is that we’re medical detectives. Instead of just treating symptoms, we ask: “Why is your B12 low in the first place?”

Diet Matters, But It’s Not Everything

If you’re vegan or vegetarian, you’ve probably heard you need to supplement B12 since it’s primarily found in animal products. That’s absolutely true! But here’s what many doctors miss: even meat-eaters can be B12 deficient.

In fact, recent research shows that about 15% of people have genetic variations that affect how they process B12. You could be eating plenty of B12-rich foods but still not getting the benefits!

The Digestion Connection Most Doctors Miss

Here’s something fascinating: your stomach needs to produce adequate acid and a special protein called “intrinsic factor” to absorb B12 properly. This complex absorption process can go wrong in several places:

  • Stomach acid issues: Taking acid reducers or antacids? You might be unknowingly blocking B12 absorption. Many of my patients are surprised when their heartburn medication turns out to be connected to their fatigue!
  • H. pylori infection: This common stomach bacteria can damage cells that produce intrinsic factor. According to a 2024 study in the Journal of Clinical Medicine, treating H. pylori can significantly improve B12 status in many patients.
  • Inflammatory bowel conditions: Crohn’s disease, celiac disease, or even mild gut inflammation can all interfere with B12 absorption.
  • Aging: As we get older, our stomach naturally produces less acid. That’s why about 20% of adults over 60 show signs of B12 deficiency even with normal diets.

 

Beyond Basic Testing: Getting the Full Picture

Have you ever had a doctor check your B12 levels? Most standard tests only look at serum B12, which doesn’t tell the whole story.

In my practice, I often see patients with “normal” B12 blood tests who still have deficiency symptoms. That’s why I recommend these additional markers:

  • Methylmalonic acid (MMA): This compound rises when your body doesn’t have enough active B12, making it a more sensitive indicator of true B12 status
  • Homocysteine: Elevated levels can signal B12 deficiency and independently increase risks for heart disease and cognitive decline
  • Holotranscobalamin (Active B12): Measures the biologically active form of B12 that your cells can actually use

When Sarah came to me, her previous doctors had only checked standard B12. When we tested her MMA levels, they were significantly elevated—confirming she truly was deficient despite “borderline normal” B12 results.

 

Personalized Treatment: Beyond One-Size-Fits-All Pills

Once we identify your unique root causes, we can create a targeted treatment plan that actually works. This might include:

Strategic Supplementation

The form of B12 matters tremendously! There are several types:

  • Methylcobalamin: My preferred form for most patients, as it’s already in an active form
  • Hydroxocobalamin: Excellent for patients with certain genetic variations
  • Cyanocobalamin: The common form in most supplements, requires conversion in the body

For patients with severe deficiency or absorption issues, I often recommend starting with B12 injections (typically 1000 μg) every few days for 2-3 weeks, then transitioning to maintenance doses.

One exciting development from recent research: combining B12 with specific nutrients like folate and trimethylglycine can enhance results dramatically, especially for patients with cognitive symptoms or mood concerns.

Healing Your Digestive System

Remember, supplements are just part of the solution. We need to fix the underlying absorption issues:

  • Testing for and treating H. pylori infections
  • Supporting stomach acid production naturally
  • Using specific probiotics that research shows can improve B12 absorption
  • Healing intestinal inflammation with targeted nutrition and anti-inflammatory protocols

Lifestyle Medicine for Better Absorption

Did you know that chronic stress can reduce your stomach acid production? Or that certain medications can deplete B12 over time?

In our practice, we look at:

  • Stress management techniques proven to improve digestion
  • Medication reviews to identify potential nutrient depletions
  • Sleep quality optimization (poor sleep affects nutrient absorption!)
  • Movement practices that enhance circulation and nutrient delivery

 

Real Results: Life Beyond Deficiency

When we address B12 deficiency at its roots, the results can be truly life-changing. My patients report:

  • Mental clarity returning after years of brain fog
  • Energy levels they haven’t experienced since their youth
  • Improved mood stability and emotional resilience
  • Resolution of mysterious neurological symptoms
  • Better sleep and digestion

Sarah, the patient I mentioned earlier, eventually discovered she had both pernicious anemia (an autoimmune condition affecting B12 absorption) AND was taking a medication that depleted B12. Six months after starting her personalized protocol, she ran a 10K race—something she never thought possible when she first came to see me.

 

Your Next Step: Becoming Your Own Health Advocate

If you’re experiencing unexplained fatigue, brain fog, mood changes, or other mysterious symptoms, here’s what I recommend:

  1. Ask for comprehensive testing beyond just standard B12 levels
  2. Look for a provider who asks why you might be deficient, not just what supplement to take
  3. Keep a symptom journal to track how you feel as you address potential deficiencies
  4. Remember that healing takes time – B12 stores build up gradually, and nerve tissue heals slowly

The journey to optimal health is personal, and addressing nutrient deficiencies is often a missing piece of the puzzle. By tackling vitamin B12 deficiency at its roots, you’re not just treating symptoms—you’re rebuilding the foundation for lasting energy, mental clarity, and overall wellbeing.
